Topic 1: What Is the Window of Opportunity?
HS is a chronic inflammatory disease in which repeated or persistent inflammation can be followed by tunnels, scarring, anatomic distortion, and functional impairment. The window-of-opportunity concept proposes that adequately controlling inflammation earlier in the disease course may improve response and reduce cumulative damage compared with waiting for advanced structural disease.1
The evidence is clinically important but still evolving. A retrospective multicenter cohort found that a therapeutic delay longer than 10 years was associated with a lower likelihood of response to adalimumab at week 16. This association supports earlier recognition and appropriate treatment, but it does not prove that a specific time cutoff applies to every patient.2
| Use careful language: Early treatment may improve the opportunity to control inflammation before additional irreversible damage occurs. It should not be presented as a promise that progression will be prevented or that all early disease requires biologic treatment. |
