Topic 8: Build an Age-Appropriate Longitudinal Plan

A pediatric plan should support both the adolescent’s growing autonomy and the caregiver’s practical role. The balance will vary by age, developmental capacity, family context, local law, and the treatment being considered.

Adolescent role Caregiver role Clinical team role
Describe symptoms, priorities, and acceptable options Support appointments, medications, supplies, transportation, and advocacy Protect confidential time and explain what information must be shared
Learn early signs of a flare and how to seek help Avoid blame and notice functional or mood changes Provide a written plan and realistic follow-up pathway
Participate in decisions at an appropriate developmental level Help with continuity without speaking over the adolescent Coordinate dermatology, primary care, mental health, school, and other specialists when needed
