Topic 3: Look in Characteristic Locations

HS most commonly occurs in intertriginous and friction-prone areas, including the axillae, inguinal folds, inner thighs, genital and pubic regions, perineal and perianal areas, buttocks, inframammary folds, and abdominal folds.¹

Distribution can vary by patient and sex. More than one region is typically affected, and patients may not voluntarily disclose lesions in sensitive areas.

Examination should be guided by the history and conducted with consent, appropriate draping, and a chaperone when indicated. If a complete examination is not possible during the initial visit, the clinician can ask about symptoms elsewhere and arrange appropriate follow-up.

Suggested patient question: Some people who have recurring lumps in one skinfold also experience them under the arms, beneath the breasts, around the groin, between the buttocks, or in other areas where skin rubs. Have you had anything similar elsewhere?
